  1. Your Business Registration Number and the full legal name of your company
  2. Company website (if applicable)
  3. Brief description of your company’s activities
  4. Company Mailing Address
  5. Evidence of Liability Insurance; minimum $2 million
  6. Primary contact person and contact information (name, email, and phone number)
